Fatality Email Template
Colleagues,
As I’m sure you’ve heard we sadly had a tragic accident on [DATE] at approximately [TIME] on one of our [LOCATION]. During [OPERATION], an individual working for one of our service providers, was fatally injured. No note will convey the impact such an event has on loved ones, family, friends and colleagues, but I find it important that I let you know my initial reflections by way of this note.
Our thoughts and prayers go out to the family and friends of the decedent – a terrible loss. We are working together with our contractor to take care of staff and offering counselling services to those effected and we have stood down field operations in [LOCATION] and [OPERATION] globally.
An investigation has been initiated and the team is on site.
While we will not pre-empt the outcome of the investigation, I would ask that all of you reflect on the following:
- When change occurs to work that is planned, however minor the change, how do you ensure the work continues safely?
- How do you ensure that people stay out of the Line of Fire?
- Are you doing proper hazard recognition?
- How do you know the conditions and systems in place to ensure work is being done safely?
We are one global [OPERATION] realted community and this tragic loss is felt by us all.
We will take the learnings from this but until then, please pause to have the conversation within your team on how you’re managing risk and change.
