Field Visit Guideline

Below are the considereation for the Field Visit based on lessons learned:

1. Visiting Team PPE

• Gloves

• Safety Glasses

• Safety Boots

1. Visit time:

• Choose the day not to conincide with Crew Changes of neither the crew nor RMs

1. Transportation:

• Get Night Driving permission before the field visit

1. Visit Agenda

• Debrief, aggree on roles and targets

• Visit to the site

• Closing meeting with RM to highlight the most important findings

• Visiting Team to feedback their findings with respective Responsible Person and Deadline for closeout

1. Required Tools

• Camera

• Notepads

• PTW for using camera in Zone 0 and Zone 1

1. Personnel

• HSE Advisor to be available to do cross audit. HSE of one unit to participate on the audit to another Unit
