Asset integrity process safety incident that occurred on April 13th, 2025, involving a fire in an oil dump tank during a well displacement operation. The company's fire team successfully extinguished the blaze, preventing further escalation. Notably, the incident resulted in no injuries to personnel and no environmental damage. An investigation is currently ongoing to determine the cause of the fire, and this information is being shared with HSE team leaders to inform their teams.
Presence of flammable hydrocarbon vapors inside the dump tank that may have ignited.
Ignition source such as static electricity, open flame, or hot surface near the tank.
Inadequate vapor venting or pressure relief, leading to buildup of flammable atmosphere.
Poor grounding or bonding during fluid transfer, increasing the risk of electrostatic discharge.
Nearby equipment with hot surfaces or exhausts acting as an ignition point.
Potential equipment malfunction (e.g. valve failure or overfilling) leading to unexpected hydrocarbon release.
Install and regularly maintain vapor recovery systems and pressure relief devices on dump tanks.
Ensure strict control of ignition sources near tanks — including enforcing hot work permits and using intrinsically safe tools.
Ground and bond all equipment properly during displacement operations to eliminate static discharge risks.
Conduct regular inspection and maintenance of dump tanks, including checking for leaks, corrosion, and vent blockages.
Perform a fire and explosion risk assessment before beginning displacement or fluid handling operations.
Enhance training and awareness programs for crews involved in high-risk operations, including emergency response readiness and process safety practices.
