DESCRIPTION OF THE INCIDENT
A male worker became trapped under a forklift while performing work duties at the facility. Emergency services were notified shortly after 12:30 p.m. Paramedics attended the scene and pronounced the worker dead at the scene.
REGULATORY NOTIFICATION
The Ministry of Labour, Immigration, Training and Skills Development (Ontario) was notified of this fatal incident and initiated investigation procedures.
