During rigging down for rig move operation from UG-0333 to UG-0338 well, Assistant Driller (AD) named Phani Kumar went to remove eye wash station, which was positioned at the end of rig floor on a metal platform. Once he reached eye wash station, the metal pproceeded to remove an eye wash station, which was positioned at the end of rig floor on a metal platform.
The metal platform was already unpinned and once the AD reached the eyewash station, the metal platform became unbalanced and fell down on the ground along with the AD from 13.7 meters. During the fall, it is concluded the metal platform flipped and landed over on the AD. Rig crew proceeded to release AD from platform and gave him first aid and further the AD was transported by Rig Emergency Vehicle and medic on board to “Adan” hospital. Later on, the AD passed away and was declared deceased at “Adan” hospital.
First Aid given at site, AD transferred to hospital for further examination/treatment (passed away in hospital). Incident area secured and preserved for investigation.
