- *Proper ppe not worn for hot work activities
- *using unsecured drums as a standing platform
- *compustable and flammable meterials are too close of hot works
- *Chemical spillage on ground
- *cleaning clothes by preasurized air hose
- *chemical drums are not properly stacked and labelling is not in place
- *improper handling of meterials

1. Manual Heavy load lifting – ergonomics.
2. Gas welding near flammable liquid – fire / explosion
3. Cutting without protection- physical hazard
4. Trip in the cables – trip
5. Liquid spillage on floor- slip
6. Ladder should be used for height work-related fall
7. One person using high pressure air on body- physical hazard

- High pressure air being used to clean coverall.
- Oil spilled on floor.
- + Cable laying on water.
- + Water being spilled from Hose reel hose cabin.
- Hot Work being carried out near the flammable materials.
- Heavy load being attempted to lift.
- Using drum as working platform to replace the lighting tube.
- Using grinding machine near the flammable materials without wearing hand glove , apron , face shield and spark hitting and reaching on flammable drum.
- + No fire extinguisher at workplace.
- Flammable materials were not kept designated location.
- + No signages, emergency contact number, evacuation route provided.
